Family Care Connect Director
The Family Care Connect Director is responsible for oversight of all program and services supporting the needs of care partners who have a loved one diagnosed with a neuro degenerative disease while insuring alignment with SJJCC’s mission and strategic goals.
ESSENTIAL DUTIES + RESPONSIBILITIES:
- Maintain a comprehensive understanding of the needs of care partners and best practices in program delivery.
- Strategic visioning, developing, and implementing programs and services based on the community’s needs while ensuring high-quality/cutting edge support. Programs such as support groups, educational workshops, memory cafes, care partner dinners, symposium.
- Develop new programs such as a wellness/self – care programs for care partners
- Complete care partner assessments to understand needs and develop a proactive plan of action while entering this data into a software system to track data.
- Create and facilitate a care partner committee to support department goals
- Design, evaluate, and expand programs for care partners with a growth mindset.
- Build and maintain relationships with community partners to act as mutual referral source
- Track and analysis data to inform program decisions, improvements, and growth opportunities
- Oversight of monthly financials and department budget
- Public speaking engagements, tabling at conferences and networking.
- Hire Support Staff to help achieve department goals
- Facilitate ongoing staff meetings and trainings
QUALIFICATIONS:
- 3-5 years supervisory experience working in comparable setting.
- LMSW / LCSW, Mental Health Counselor
- Experience working with individuals with Dementia and other neuro degenerative illnesses and their families
- SIFI Certified
- Proficient in Microsoft Office
- Communication skills including strong interpersonal skills
- Exhibit patience, initiative, enthusiasm, resourcefulness and strong attention to detail.
- Ability to provide leadership and motivation to staff and work effectively with diverse groups of people.
